Hospital Social Worker

This Social worker will be required to work as a key member of the Hospital Social Work Team. To provide holistic, complex assessment and care planning for patients of the general hospital, which will include intermediate care services and the community to ensure equitable appropriate access to services for clients. To provide assessment, advise and support to carers. To put the client at the centre of care planning undertaking where appropriate multi disciplinary assessment and planning. To provide training, advice and support for colleagues within the hospital social work team.

The appointed Social Worker will be GSCC qualified and have a current Enhacced CRB.
