Our Client has a number of vacancies for Qualified Social Workers to join various adult care management roles within Staffordshire and surrounding areas.
In these role's you will be undertaking the full range of tasks associated with the Assessment, Care Planning and Reviews. These tasks will include:-
•Undertaking a duty role as necessary.
•Initial assessments in order to prioritise level of intervention.
•More detailed assessments ensuring a needs led approach within the spirit of ‘Our health, our care, our say’, other current legislation, and to act as care coordinator (named worker) as required.
•Ensure promotion and assistance for self assessment and self directed support.
•Accurately recording peoples and their primary Carers needs.
•The writing of clear and concise Care/Support Plans coordinating the involvement and assessments of other professionals, and others, as necessary.
To monitor and review Care/Support Plans as per Operational Guidelines involving all relevant people in any changes to the Care Plan. Where necessary, to undertake comprehensive re-evaluation.
