Hospital Social Worker

Our client requires an Experienced Qualified Social Worker to work in an Adult Care Management team 65 +.
To be considered for this role, you will need to have experience working in a hospital and community environments and have excellent communication skills.

Duties will include preliminary assessments, care planning, hospital discharges and providing general information on how and where to apply welfare benefits.
This is a temporary position for a minimum of three months.
This is a particularly challenging role in which the right candidate will be working principally independently.
